This fee is payment for your Temporary Food Sevice Application. The $60 fee is assessed by the Cobb & Douglas Public Health Department.
Online Payments will incur a 5% processing fee that's included in the total amount charged.
PAYMENT MUST BE MADE IN FULL TO ENSURE YOUR INCLUSION ON THE DAY OF EVENT VENDOR INSPECTION LIST.
Food Service Permit Fee: $60 + $3 online processing fee = $63.00 Total

- Print, complete, and sign the Food Services Vendor Inspection Application (attached)
- Write your check in the amount of $60 for the Permit Application, and make it payable to Cobb & Douglas Public Health Dept.
- Staple check to the application
- Mail the completed & signed form with the check to: Ricketts Rhodes Events, c/o SpringsFest, 4171 Marietta Street, Suite 300A, Powder Springs, GA 30127
- Submit your $60 Permit Application payment (note there is a 5% processing fee to submit the payment online)
- After the payment is processed, email the completed & signed Permit Application form to SF4th@rickettsrhodes.com
In the Subject Line put Food Vendor Inspection Form_ Your business name.
In the Body of the Email, please confirm the date your payment was made.
- An email confirmation will be sent to you when the payment and application are received.
No refunds will be given for cancellation of the event due to inclement weather or circumstances beyond the organizers control.
